To participate in intercollegiate athletics at Saint Anselm, the Sports Medicine staff requires student-athletes to complete several forms. For your convenience and for improved care by the staff, these documents are completed online through Athletic Training software.

All student-athletes who are currently prescribed a medication by their physician that is banned by the NCAA MUST provide all medical exemption documentation via the Medical Exemption Form. Any further questions can be answered by the Saint Anselm College Sports Medicine staff.
Student-athletes are required to download and read the Sports Medicine Sickle Cell Testing informative letter. The Sickle Cell Testing results form must also be completed.
All first-year student-athletes must register at the NCAA Eligibility Center to be cleared for initial eligibility. Please remember to contact your school to forward your final grades and transcript to the Eligibility Center upon graduation. To determine the status of documents sent or to check on your eligibility status, contact the NCAA at (877) 262-1492. The NCAA Eligibility Center notifies you and Saint Anselm when you meet all requirements.
